In April 2011, two workers were killed when a large containment basin collapsed at the Gatlinburg (Tenn.) Wastewater Treatment Plant, spilling more than a million gallons of wastewater into the Little Pigeon River. On the morning of Feb. 23, another worker died in a trench collapse during work to repair and rebuild from last year’s accident. News reports were just emerging, and personnel from the Tennessee OSHA Safety and Health Division office in Kingsport were investigating the incident.
